This letter confirms the July 14, 2013 verbal notification provided to you by Chicago District Deputy District Manager (DDM) Dr. Tamara Davis of Food Safety and Inspection Service’s (FSIS) decision to place the suspension of slaughter operations in abeyance at your establishment, located at the above address. This decision is based upon your responses to the Chicago District Office on July 13 and 14, 2013.

This letter confirms the July 11, 2013, verbal notification provided to you by Dr. Tamara Mayberry-Davis Deputy District Manager, of the Food Safety and Inspection Service’s (FSIS) decision to suspend slaughter inspection at your establishment, located at the above address, as specified in Title 9 of the Code of Federal Regulations (9 CFR) 500.3 (b). This action was taken because of your establishment’s inability or unwillingness to effectively implement humane methods of slaughtering and handling of animals under the Acts, as outlined below, and in a manner that complies with FSIS regulatory requirements. This action is based on FSIS’ findings that your establishment failed to comply with the requirements of 9 CFR 313.30 (a) (1), 313.30 (a) (3), 313.30 (a) (4), 313.30 (b)(!), and3l3.30 (b) (3).

This letter confirms the verbal notification the Food Safety and Inspection Service (FSIS), Deputy District Manager (DDM), Dr. Mounira Naguib on Novemeber 13, 2013, of the suspension of the assignment of inspectors for the slaughter operation at your establishment, Zabiha Halal Meats Establishment number M45105. This action was initiated, in accordance with the Rules of Practice described within Titles 9 Code of Federal Regulations (9 CFR) 500.3(b) after FSIS determined that your establishment failed to handle animals humanely in accordance with 9 CFR 313.16(a)(1).
