Citation: While performing a ten bird Food Safety test on line # [redacted] at approximately 05:32hrs, I observed fecal on bird #7. I notified [redacted] of the 03J01 noncompliance. The bird was small with disjointed hips and torn skin. The fecal was missed by the brushes in the final washer due to these conditions. The fecal, greenish in color and 1/4 inch in size, was located on the breast. The hangers, sorters and final trimmer were counseled on the removal of birds with these conditions and have them reworked at the trim table. A HACCP retest was at 05:41hrs and found to be acceptable. The affected bird was washed at the product wash station and placed on the line prior to the [redacted] rinse. No regulatory action was taken at this time because the plant followed their HACCP corrective action plan. The 03J02 was performed and found to be acceptable. There was a yellowish brown smear approximately 1/4 inch by 1/4 inch in size, located just below an attached cloacae, near the kidney area of the bird. was informed of the 03J01 noncompliance for the plant’s corrective actions. I was informed that the sprayers on the Washer were adjusted. A HACCP retest was performed at 0233 hrs and found acceptable. The affected bird was rewashed and placed back into production in front of the system. Citation: Before the start of the plant’s daily operation, and after PSSI’s check, in performing Pre-Operational sanitation, of 21 units checked, there were 2 SSOP non-compliances. In 2nd processing, in Marination, at IU# 14, the injector needles, had product residue inside 3 of the 5 needles checked. Unit was rejected with US Reject tag # B25879908, at 21:54. Needles were replaced and rechecked at 22:20 and released. In 3 & 4, at IU 19, the Fat Sucker wheel, had 3 units with product residue on the top lip right where the birds are placed against. Unit was rejected with US reject tag #B25879419 at 22:08. Cleaned and released at 22:15. was present at each area and immediately informed of the non-compliances. There was no product in the areas of inspection. 416.1 States:”Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to insure product is not adulterated.

Citation: While I was performing inspection duties in the giblet area of second processing at 0413 hrs, I observed several pieces of intestines and fecal material in a combo that the livers were being harvested into. I applied US Reject tag #B 25 879971 to the combo and immediately informed [redacted] of my findings. An employee removed the affected combo and replaced it with a clean combo and liner. Prior to any corrective action being performed, the plant started to harvest livers into the second combo. I informed [redacted] that there was no corrective action taken to ensure any further product contamination. I applied US Retain Tag # B 25 879972 to the second combo of livers. The plant’s immediate corrective action was to turn the liver pumps off. The affected flume pipes and liver belts were cleaned and sanitized. All of the affected product was washed with [redacted] water. [redacted] re-inspected the product at 0746 hrs and released the product. Approximately 585lbs. of livers were involved. The affected liners were thrown away. The plant’s preventative measure was to counsel all employees involved. Section II A. of the plant’s SSOP program states, [redacted].” 9 CFR 416.1 states, “Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.”

Citation: While performing a retained water test on chiller # 2,at 23:49hrs, I observed that the retained water had exceeded the maximum allowed, which is 12%, with a result of 12.40%. A reweighing of the test birds, 15 minutes later, found that they were now within the allowed retained water rate, establishing the drain time. A retest was performed at 01:35 and found to be acceptable. All associated product was retained and allowed to drain, as per the plants written procedure for failed retained water tests, bringing them back within acceptable limits.

Citation: While performing Early Preoperational Sanitation Inspection, I observed 1 SSOP noncompliance in the Evisceration Department. One of the liver flumes for line 2 had a piece of brown colored product residue, approximately 1/4 inch round in size, and a piece of fat, approximately 1/4 inch round in size, in it. Two other flumes that were there had small unidentifiable foreign material in them. The particles numbered in the 20’s, and were from specks to 1/8 inch in size. [redacted] was informed of the SSOP noncompliance and I applied US Reject Tag # B25879912 at 1939 hrs.[redacted] had the flume pipes cleaned and I released them at 1943 hrs. While performing Regular Preoperational Sanitation Inspection, prior to the start of operations, and after QA and PSSI’s inspection, I observed 1 SSOP noncompliance of the 24 units inspected. In Area 5, Unit 7, the # 3 rehang belt, the teflon slides that support the belt near the middle, on the underside, had a white greasy substance on them. If you would rub your fingers along the slide the substance would build up on your fingers to as much as approximately 1/8 inch thick. [redacted] was informed of the SSOP noncompliance and I applied US Reject Tag # B25879911 at 2116 hrs. [redacted] had the unit recleaned and I released it at 2125 hrs. 9 CFR 416.1 states, “Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.”

Citation: At 1355 hrs. while performing a Food Safety check on line #2 I observed feces on one bird of the ten bird sample. The feces was greenish in color, located in the kidney area. I notified [redacted] and she observed the 03J01 fecal non-compliance. The immediate corrective action, guts were removed from the drawer spoons and the spoons were cleaned off. Further preventive measures, maintenance adjusted water sprayers on the drawer to ensure that guts and fecal are being removed from the spoons. The bird was taken to the product wash station, the fecal removed, and the bird was returned to production before the (anti-microbial) system. I monitored the HACCP retest being performed and the test was found acceptable at 1407 hrs. No regulatory control action was taken on any product because the plant followed their HACCP corrective procedures. The 03J02 procedure was performed and found acceptable. Citation: While I was performing inspection duties in the [redacted] 2 area at 0158 hrs, I observed an employee using a high pressure hose to clean the floor at break time. There were approximately ten boxes in the area that were getting wet on the inside from the overspray. I applied US Retain Tag # B25 879425 to the boxes. The boxes for product are not lined so therefore, are product contact surfaces. I immediately informed of the [redacted] SSOP non-compliance immediate corrective action was to discard the boxes. The issue of the plant employees using high pressure hoses to clean the floors at break, was discussed at the PBIS meeting on 09/28/2010. Citation: At 1414 hrs. while performing inspection duties in the Rehang Area, I observed a large stainless steel tank which was at least 3/4 full of whole birds. The tank was under the return/drip line outside of hanging belt # 3 at the exit end of Chiller # 1. I observed numerous black specks up to 1/4 inch in size on nearly all the birds I could see in the tank causing direct product contamination . These black specks appeared to be coming from the railing above the tank of birds. US Retained Tag # B 25 879771 was applied to the tank of birds. I informed [redacted], and he observed the 01C02 SSOP noncompliance. [redacted] immediate corrective action taken was to remove the tank from under the line to the product wash station for the birds to be washed. I and [redacted] observed the birds being washed. Citation: At approximately 1700 hours, while performing inspection duties, in the 3 dumping belt area, I observed a plant employee pick up several carcasses off of the floor and toss them onto the production belt containing clean product. was notified of this 01C02 SSOP noncompliance and opted to discontinue operations because it was close to the end of her shifts production. The supervisor’s immediate corrective action/preventative measures was to retrain/discipline the employee on the proper handling of the product to avoid direct/indirect product contamination and to stop production and rewash all affected product on the belts. In doing so, some of the carcasses that fell from the top belt onto the rehang belt was tossed into a vat of clean carcasses which in turn contaminated more product. I had the employee stop returning the birds to the belt. The brown spots were on as many as 15 birds. I also observed the same brown colored spots were on the top edge of the tank and the edge of the rehang belt frame. I informed [redacted] of the SSOP noncompliance and retained the tank of whole birds with US Retain tag # B25879428 at 2306 hrs. I also observed that the birds on the end of the # 3 rehang belt had the brown colored spots on them. It was on as many as 15. This was also shown to [redacted]. She had the drip line stopped and the birds removed to be rewashed. She informed me that she had the rail and guide wheels in the area wiped down.[redacted] had the affected birds rewashed and I reinspected and released them at 0048 hrs. She informed me that the tank was rewashed and sanitized.[redacted] informed me that her preventative measures are to have blowers installed on the drip lines and to send an E-mail to the sanitation supervisor to have the tops of the high guide wheels cleaned. The last SSOP check was performed at 2305 hrs and found acceptable. The establishments immediate corrective action was to adjust the sprayers on the machine to better clean the probes and pinchers. A HACCP retest was performed at 22:14hrs and found to be acceptable. The affected bird was washed at the product wash station and placed on the line prior to the rinse. Citation: Before start of operations, performing Early Pre-Operational sanitation, in the Redwater room, I had several elbows removed off the giblet system that were used for each giblet. The elbows for the gizzard system had product residue in them. [redacted] was observing, and was informed of the SSOP non-compliance. Unit was rejected at 19:05, with US Reject tag #B25879416. Unit was cleaned on indication and released at 20:03. After PSSI’s inspection, and QA’s inspection, before start of operations, of 24 units checked there were 2 SSOP non-compliances. In second processing, in Marination, IU#14, the injector pump, had product residue oozing from the seals, and inside the pump along the line and the seal, and had leaked under the pump and had a brown puddle on both sides under the pump that was approximately 6 X 8 inches and twice that size on the other side [redacted] was observing, and was informed of the SSOP non-compliance. Unit was rejected at 21:33, with US Reject tag #B25879417, cleaned on indication and released at 21:41. At [redacted] 1, IU#22, Fat Sucker, had product residue on 4 of the units, and residue above 6 of the units spattered in a semi-circle above each one. [redacted] observed the finding, and was informed of the SSOP noncompliance. The inside of the metal shield had product spray over 2/3 of it’s interior. Unit was rejected with US Reject tag #B25879418 at 22:01, cleaned on indication, and released at 22:15. In area 5, unit #35, the neck chiller, there were 8 to 10 pieces of meat on top of the incline belt that leads to the collection vat. I applied USDA reject tag # B-25-879899 to the unit at 22:35. In Area 4, Unit 13, the neck breaker machine, there was meat and fat residue in the concave piece of the machine that cuts the neck from the bird. It was in 8 of the cutters that I inspected. It covered areas as big as approximately 1 inch long by 1/2 inch wide and 1/4 inch thick. [redacted] was informed of the SSOP noncompliance and I applied US Reject tag # B25879896 at 2051 hrs. The unit was recleaned and I released it at 2108 hrs. In Area 6, Unit 14, the injector needles had product residue in 10 of the 14 needles that I inspected. I informed [redacted] of the SSOP noncompliance and applied US Reject tag # B25879897 at 2142 hrs. [redacted] had maintenance change the needles with cleaned and sanitized needles. I reinspected the unit and released the unit at 2218 hrs. This unit was also inspected by QA and found acceptable. In Area 10, Unit 3, the bagger for [redacted] 4, there was a build up of product residue between and around a piece of plastic tubing that is fastened to the flap where the bags are sealed by the machine. The flap and piece of tubing are approximately 18 inches long. The residue was black, brown and white in color. [redacted] was informed of the SSOP noncompliance and I applied US Reject tag # B 25879898 at 2208 hrs. Citation: At 1240 Hrs. while performing a Food Safety on line #1 feces was observed on one bird of the ten bird sample. The feces was greenish in color, located in the kidney area. was informed and observed the 03J01 fecal non-compliance. The immediate corrective action, the bird was taken to the product wash station, the fecal removed, and the bird was returned to production before the (anti-microbial) System. Maintenance lowered the hip lifters on the vent machine so that the blades would not go too deep inside the bird. I monitored the HACCP retest being performed and the test was found acceptable at 1257 Hrs. No regulatory control action was taken on any product because the plant followed their HACCP corrective action procedures. Further preventive measures will be to turn on and adjust extra sprayers to spray on the inside and outside of the birds for the remainder of the day to prevent reoccurrence. Citation: While performing VI duties in second processing at approximately 2330 hrs, I observed brown colored spots on whole birds in a stainless steel tank, which a plant employee was tossing back onto the # 3 rehang belt. It was on as many as 15 or more birds. I had the employee stop returning the birds to the belt and observed that the same spots were present on the edge of the rehang belt. While observing the belt, one could see the spots appear on the belt itself. The drops appeared to be coming from the drip line and rail. I had the employees stop hanging and informed [redacted], of the noncompliance. I applied US Retain Tag # B25879754 at 2332 hrs to the tank of whole birds. [redacted] was then notified of the 01C02 noncompliance. The birds at the end of the rehang belt also had the brown colored spots on them. The spots numbered in the hundreds on 30 or more birds. All the birds on the belt were placed in a tank and I retained them with US Retain Tag # B25879426. I also rejected the rehang belt with US Reject tag # B25879427at 2340 Hrs. [redacted] had the belt and drip line rail recleaned and I released it at 0010 hrs. The affected birds were rewashed and I released them at 0328 Hrs. The unit was recleaned and released at 2225 hrs. In Area 8, Breast Debone, Unit 35, there were pieces of meat and fat on the white belt of the sizer. One piece of meat was approximately 3 inches long by 1 inch wide. A piece of fat was gay in color and was approximately 2 square inches in size. On the ends of the teflon slides that bring the product from the [redacted] Cutter, there was meat and a white fatty residue. It was on 3 of the 4 slides and covered approximately 80 % of the end of each slide. [redacted] was shown the SSOP noncompliance and I applied US Reject tag # B25879751 at 2235 hrs. The unit was recleaned and I released it at 2242 hrs There was no product in any areas of my inspection. In 2nd Processing, Breast DeBone, IU# 47, had a portable belt stand that had product residue in several of the belt sections(numbering 20+), and product residue on the framework holding the belt. This was shown to Jason and the unit was rejected and tagged with US Reject tag #B25879406 at 21:42. Unit was cleaned on indication, and released at 21:47. I applied retain tag #B-25-879795 on the pallet of products. QA also placed their retain tag on them. 381.169(a) Butter alone, or solutions of poultry broth, poultry stock, water, or edible fats, or mixtures thereof, in which are included functional substances such as spices, flavor enhancers, emulsifiers, phosphates, coloring materials, or other substances, approved by the Administrator in specific cases, may be introduced by injection into the thick muscles (breast and legs) of ready-tocook poultry carcasses and may be introduced by injection or marinating into any separate bone-in part therefrom, for the purpose of providing a basting medium or similar function. The ingredients of the added materials and the manner of addition to the products must be found acceptable by the Administrator, in all cases. The introduction of the added materials shall increase the weight of the processed product by approximately 3 percent over the weight of the raw product after washing and chilling in compliance with § 381.66. The weight of the added materials introduced into the poultry products as provided in this paragraph shall be included as part of the weight of the poultry for purposes of the net weight labeling provisions in § 381.121(b).

Citation: While performing pre-operational sanitation inspection, prior to start-up and after [redacted] had cleared the area, 3 SSOP noncompliances were found. 1 during early pre-op and 2 during regular pre-op out of the 23 units checked. During early pre-op, 10 to 12 pieces of what appeared to be meat and fat , all about 1/4″ to 1/2″ in size, were found in the bottom of chiller #2. I rejected the unit and attached USDA reject tag # B-25-878564 at 19:28. The unit was cleaned and released at 19:48. Then on regular pre-op I observed a 1 1/2″ piece of fat and some specks of rust on top of a shield that covers the wheels for the drip line. The birds on this drip line hit this shield, therefore it is a product contact surface. I rejected unit #7, area #5, and attached USDA reject tag # B-25-878565 at 20:51. The unit was cleaned and released at 20:59. In breast debone , area #8, unit #1, the drop chute for the first cone line, I found about 8 pieces of fat and meat each about 1/4″ in size. I rejected the unit at 21:53 and attached USDA reject tag # B-25-879759. The unit was cleaned and released at 21:58. Citation: While performing Preoperational Sanitation Inspection, prior to the start of operations and after QA and PSSI’s inspection, I observed 2 SSOP noncompliances of the 23 units inspected. In Area 10, unit 15, the slide supports for the bottom of the grader belt had a build-up of product residue on them. When you would run your fingers along the slides the residue would coat your fingers with white fatty substance and pieces of product residue. It was along a length of approximately 6 feet of the slides. There was also product residue in between where the 2 top teflon guides meet each other. This was present on the inside and outside teflon guides. [redacted] was shown the SSOP noncompliance and I applied US Reject Tag # B25879768 at 2138 Hrs. [redacted] had the unit recleaned and I released it at 2147 Hrs. In Area 12, Unit 17, there was a fatty, brownish, fatty feeling residue on the inside of the chute under the product saw. There was also a piece of fat approximately 2 inches long by 1 inch wide hanging from a small water line that runs through the chute opening. In the same area there is a square tube running across the opening with 2 small waterlines through it that was full of fat.[redacted] was shown the SSOP noncompliance and I applied US Reject Tag # B25879767 at 2203 hrs. [redacted] had the unit recleaned and I released it at 2210 hrs. There was no product in any areas of my inspection. In Area 3, Unit 11, the gizzard belt that takes the product to the skinner machine, had a white build up on the slides that support the belt on the underside. The build up was also on the framework and cross braces. It could be easily scratched off with one’s finger. It was as thick as 1/8 inch in numerous places. I informed [redacted] of the SSOP noncompliance and applied US Reject tag # B25879769 at 2035 hrs. [redacted] had the unit recleaned and I released it at 2053 hrs. This unit was also inspected by QA and found acceptable. In Area 8, Units 9 and 10, the cone lines in breast debone, there was a white, fatty feeling residue on the slide supports for the belts that move product from the cone line to other parts of the department. In was along a section of approximately 3 feet on each of the 2 belts in the area. There was also numerous pieces of meat and fat on the belts, ranging in size from 1/8 inch to as big as 1 inch. The SSOP noncompliance was shown to [redacted] and I applied US Reject Tag to the unit at 2152 hrs. There was also pieces of product residue on the floor and on the step platform that goes over the one belt. This was also pointed out to [redacted] He had the entire units recleaned and I released it at 2218 hrs. There was no product in any areas of my inspection. This tank was under the return/drip line outside of hanging belt #3 at the exit end of Chiller # 1. There were numerous black specks up to 1/4 inch in size on the birds in the tank causing direct product contamination. These black specks appear to be coming from the railing above the tank. US Retained tag # B 25 879775 was applied to the tank of birds.[redacted] of Rehang was informed and observed the 01C02 SSOP non compliance.[redacted] immediate corrective action taken was to remove the tank from under the line to the product wash station for the birds to be washed. The tank was then cleaned and sanitized. The birds and the tank were found acceptable for release at 1254 Hrs [redacted] indicated that his further preventive action to be taken would be for all tanks under the lines would be covered and that he would also get with [redacted] to have the railings cleaned. At 1947 hrs I observed product residue inside the flume pipes for the water re-use system for evisceration lines 1 and 2. The accumulation of residue was approximately 1/4 inch thick throughout the inside of all the flume pipes. I immediately notified of the non-compliance and applied US Reject tag# B25 879801 to the unit. immediate corrective action was to have the flume pipes cleaned. At 2041 hrs I re-inspected the pipes and removed the tag. At 1953 hrs I observed the same noncompliance for the re-use system on evisceration lines The residue was inside all of the flume pipes and was approximately 1/4 of an inch thick. I immediately informed of the non-compliance and applied US Reject Tag# B 25 878482 to the unit. immediate corrective action was to have the pipes cleaned. I re-inspected the unit at 2043 hrs and removed the tag. While I was performing regular pre-operational sanitation inspection prior to the start of production and after PSSI’s sanitation inspection, I observed 1 SSOP non-compliance out of 22 units inspected. At 2143 hrs I observed product residue on the inside of a product chute for 2. These chutes are placed under the saws on the line to direct product onto the grader belt. All four sides inside of the chute had product residue on it. It was approximately 1/8 of an inch thick. I informed of the non-compliance and applied US Reject tag# B25 878545 to the unit. immediate corrective action was to have the chute cleaned. I re-inspected the unit at 2148 hrs and removed the tag. Citation: At 0621Hrs. while performing inspection duties near [redacted] #2, I observed a Vat of breast meat between [redacted] #2’s mix tank and the concrete pillar, this vat was under the Evis return line which was bent and water from the drip pan was dripping into the vat causing direct product contamination. At this time [redacted] was informed and observed the SSOP noncompliance. The vat of product was retained with USDA tag #B 25 879774. immediate corrective action was to remove the vat, have it taken to a product wash station and all product was removed and washed. This product was then placed in a clean vat with a new liner also this vat was covered with plastic to protect the product. indicated that his further preventive measures to be taken was to make sure no product was placed under this drip pan. had maintenance repair the drip pan so it was not leaking. He also indicated that he would monitor this area periodically for drips. The product was found acceptable for release at 0702 Hrs. Citation: While performing a Food Safety check on line 2 at 0422 hrs, I observed fecal material in bird #10 of the 10 bird test. There was a brownish-yellow smear approximately 1 1/2 inches long by 1/8 inch wide going down across the kidney area of the bird. was informed of the 03J01 noncompliance for the plants corrective actions. The plants immediate corrective action was to remove birds from the probes in the Washer. I observed the HACCP retest being performed at 0433 hrs, which was found to be in compliance. The affected bird was rewashed and placed back into production in front of the system. There was no regulatory control action taken on any product because the plant followed their HACCP corrective action plan. This is a repeat of a similar noncompliance, see NR # 183-2010 dated 06/20/2010. This document serves as written notification that your failure to comply with regulatory requirements of 9 CFR 381.65(e) and 417.2(c)(4) could result in additional regulatory or administrative action as described in 9 CFR 500.4.
