Violation: 416.13(b), 416.13(c), 416.15(a), 416.15(b)

Citation: The following Sanitation Standard Operating Procedures (SSOP) non-compliances demonstrate a failure by the establishment to perform an adequate pre-operational inspection and take appropriate corrective action as per the SSOP. This fails to meet the requirements of 9 CFR 416.13 and 416.15(a). The following was also observed and documented accordingly: The pre-operational record form E-1 "Processing and Sanitation Daily Record" fails to include all food contact surfaces subject to pre-operational monitoring. These food contact surfaces include the cradle used to hold the carcass while skinning and also include the steel hooks used to hang the carcasses. This fails to meet the requirements of 9 CFR 416.13(c). The same form E-1 "Processing and Daily Sanitation Record" documented "U" on 2 dates during the record review period, January 7th 2011, showed a pre-op defect for the boning table (grease). The table was cleaned/rewashed. There was employee training. The corrective action form E-2 "Corrective Action Record" included date, monitoring defect description, corrective/preventative measure and signature. On February 4th 2011, the defect identified was a dirty knife. The corrective/preventative measure was identified as "informed the employee/retrain". The corrective action form documented for February 4th 2011, failed to document a corrective action (dirty knife) to restore sanitary conditions. This is a failure to meet the requirements of 9 CFR 416.15(b). On February 10th 2011, at 9:40 a.m. the EIAO's directly observed three employees return to the process room after break. The employees washed their hands with water but the use of soap/antiseptic on their hands was not observed, as described the SSOP, page 5, paragraph B.1.c. In addition, an employee waited on a customer, made change, (handled money) and returned to quartering a beef carcass in the cooler without first washing his hands or using antimicrobial hand soap ([redacted]). These employee actions demonstrate a failure to implement the SSOP as designed. This fails to meet the requirements of 9 CFR 416.13(b). Regulation:

416.13(b) Each official establishment shall conduct all other procedures in the Sanitation SOP’s at the frequencies specified.

416.13(c) Each official establishment shall monitor daily the implementation of the procedures in the Sanitation SOP’s.

416.15(a) Each official establishment shall take appropriate corrective action(s) when either the establishment or FSIS determines that the establishment’s Sanitation SOP’s or the procedures specified therein, or the implementation or maintenance of the Sanitation SOP’s, may have failed to prevent direct contamination or adulteration of product(s).

416.15(b) Corrective actions include procedures to ensure appropriate disposition of product(s) that may be contaminated, restore sanitary conditions, and prevent the recurrence of direct contamination or adulteration of product( s), including appropriate reevaluation and modification of the Sanitation SOP’s and the procedures specified therein or appropriate improvements in the execution of the Sanitation SOP’s or the procedures specified therein.
