Citation: At approximately 0715 hours on the 2nd of February, while performing SPS verification, the following was observed: *a crack approximately 1" x 3" on the floor tile exposed pieces broken tiles and several small chipped floor tiles at the table scale and the entrance to the cooler 3; the water/fluid embedded in-between the floor layers of one floor crack; *an opening/gap approximately 3/8" on the beam wall at the sanitizer between the shaving and gutter areas; *broken in small pieces of one floor tile ranged 3" x 6" in the corner of hallway to the back of the lavatory room adjacent to the cooler 4; *opening/gap about ¼ inch to ¾ inch of on the bottom corner seal floor tiles at the entrance door to the back lavatory room hallway; [redacted], slaughter supervisor, was informed of the noncompliance. The establishment has no documentation reflecting to the aforementioned findings. This a failure of 9CFR 416.2(b)(1)(2)(3) and 416.1.

Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(b)(1) Construction. Establishment buildings, including their structures, rooms, and compartments must be of sound construction, be kept in good repair, and be of sufficient size to allow for processing, handling, and storage of product in a manner that does not result in product adulteration or the creation of insanitary conditions.

416.2(b)(2) Walls, floors, and ceilings within establishments must be built of durable materials impervious to moisture and be cleaned and sanitized as necessary to prevent adulteration of product or the creation of insanitary conditions.

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.
