Citation: At approximately 1030 hours, while performing an SPS task of the Employee’s Lunchroom, the following was observed: Approximately ten (10) white folding chairs had an appearance of grit and grime embedded unto the seat and back rest surfaces, even though nothing was transferred from touching the surface. Also, the floor was in need of a good general cleaning, with embedded dirt on the surface and wrappers throughout the eating area. Also, the room has a door to access the outside area of the roof of the building. At the time of this walk through, the door was wide open and no screen or storm door was present. With the door being opened it allowed access for outside vermin to enter the building. This lunchroom is mainly used by employees who work in the Ready-To Eat area and the threat of cross contamination is present. I notified Mr. [redacted] both orally and also with the writing of this document.


416.12(a) The Sanitation SOPís shall describe all procedures an official establishment will conduct daily, before and during operations, sufficient to prevent direct contamination or adulteration of product(s).

416.13(b) Each official establishment shall conduct all other procedures in the Sanitation SOPís at the frequencies specified.

416.15(a) Each official establishment shall take appropriate corrective action(s) when either the establishment or FSIS determines that the establishmentís Sanitation SOPís or the procedures specified therein, or the implementation or maintenance of the Sanitation SOPís, may have failed to prevent direct contamination or adulteration of product(s).

416.16(a) Each official establishment shall maintain daily records sufficient to document the implementation and monitoring of the Sanitation SOPís and any corrective actions taken. The establishment employee(s) specified in the Sanitation SOPís as being responsible for the implementation and monitoring of the procedure(s) specified in the Sanitation SOPís shall authenticate these records with his or her initials and the date.  

416.2(b)(2) Walls, floors, and ceilings within establishments must be built of durable materials impervious to moisture and be cleaned and sanitized as necessary to prevent adulteration of product or the creation of insanitary conditions.

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.
