Citation: March 15, 2011 10:15-10:35 hours While performing a scheduled preoperational, 01B02, inspection procedure in the processing department, I made the following observations: 1. Product residue adhering to the middle of the west wall, a non-food contact surface, including approximately five (5) pieces of poultry fat and muscle ranging in size from 0.5 to 0.75 inches at their greatest dimension. 2. Approximately twenty (20) dead, winged insects of the family Psychodidae, commonly called drain or moth flies, measuring approximately one eighth of an inch in length, in the reprocessing sink, and on the stainless steel grate contained therein, both direct food contact surfaces. 3. Dark brown machine grease dripping from an electrical motor housing adjacent to the west end of the boning table's lower conveyor belt and pooling in white, absorbent paper toweling on the concrete floor. 4. Rust affecting approximately two (2) square inches of the top of the motor housing adjacent to the west end of the boning table's upper conveyor belt. 5. Residue of an unknown nature, off-white in color, coating the interior, stainless steel surfaces of the two (2) pneumatic plastic bag openers, all non-food contact surfaces. I rejected the processing room and its contents by affixing the Food Safety and Inspection Service's Form 6502-1 #B39133049. These observations constitute failures to comply with the United States Code of Federal Regulations, Title 9, Chapter III, Animals and Animal Products, Sections 416.13(a) and 416.13(c). The processing department's co-manager, Mr. [redacted] was notified of the noncompliances verbally promptly. Prior to these discoveries, Mr. [redacted] had released the department and its contents to me. I noted that the establishment's preoperational inspection form had been completed, authenticated, and dated by Mr. [redacted] at 10:00 hours earlier this morning. This document serves as written notification that failure to comply with regulatory requirements may result in additional regulatory and/or administrative action(s). Past Similar NRs - Previous Ineffective Plant Actions: Failure to complete a thorough preoperational inspection of facilities and equipment as described in the establishment's SSOP plan. NR: 12-2011 dated 3/14/2011

Regulation:

416.13(a) Each official establishment shall conduct the pre-operational procedures in the Sanitation SOP’s before the start of operations.

416.13(c) Each official establishment shall monitor daily the implementation of the procedures in the Sanitation SOP’s.
