Citation: December 13, 2010 08:08 hours While performing an unscheduled operational inspection procedure in the employee welfare area, I observed five (5) desiccated chicken bones, two (2) pieces of spent chewing gum, assorted plastic refuse, and pieces of stale bread underneath and behind the vending machine. Another piece of stale bread, two (2) inches by three (3) inches in size, was observed next to the dirty laundry cabinet. I rejected the area by affixing the Food Safety and Inspection Service's Form 6502-1 #B39718925 to the door knob of the entrance to the facility. This constitutes a failure to comply with the United States Code of Federal Regulations, Title 9, Chapter III, Section 416.4(b). Mr. [redacted] in charge of general sanitation of the official premises, was promptly alerted of the noncompliance verbally. This document serves as written notification that failure to comply with regulatory requirements may result in additional regulatory and/or administrative action(s).

Regulation:

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
