Citation: Today at approximately 2010 hours, during the 2nd shift processing operating time, I observed the rusted opening gap about 1 inch wide appearing in between 2 lowest rolling gate's body-plates near the ground. The rolling gate was located next to the receiving dock office. I had reviewed the plant's Plant Improvement Program and the Open-NR file in the USDA office at this plant #31727. I found no record or document recording what I observed. I immediately notified and shown of the noncompliance to the Processing Co-Manager Mr. [redacted] Mr. [redacted] recorded what he had observed and said that he will report the noncompliance to plant manager Mr. [redacted] on the next day. Failure to meet the federal regulatory requirement of the 9 CFR 416.2(b)(3) which states: Wall, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice. This document serves as written notification that failure to comply with the federal regulatory requirements may result in additional regulatory and/or administrative action(s).
