Citation: July 18, 2010 10:10hrs While conducting an unscheduled preoperational inspection of the processing room, Inspector-In-Charge, [redacted] observed particles of rust, dust, and other unidentifiable foreign material, 1/32 of an inch in size, scattered about a two foot section of the end of the white, scale conveyor belt. Inspector [redacted] also noted that a stack of 25 beige, plastic lugs smelled of mildew. FSIS Form 6502-1 #B39133017 was applied to the conveyor belt and lugs, both direct food contact surfaces, rejecting the equipment. Processing co-manager, Mr. [redacted] was immediately verbally informed of the noncompliances. As this was a preoperational inspection, no product was involved. It was obvious that a thorough, organoleptic, preoperational inspection of this equipment had not been performed by Mr. [redacted] despite the fact that records for today indicate to the contrary. On June 30, 2010, Inspector [redacted] documented a similar Noncompliance Record (NR), #0049-2010-15201, for the plant's unsatisfactory preoperational inspection. The plant's response to this NR is as follows: "[redacted] had checked the machine but not thoroughly enough because one of the employees had told him it was cleaned. From now on he will inspect the machine himself to make sure that it is in a satisfactory condition." This document serves as written notification that failure to comply with regulatory requirements may result in additional regulatory and/or administrative action(s). 416.13(a) Each official establishment shall conduct the pre-operational procedures in the Sanitation SOP’s before the start of operations.
416.13(c) Each official establishment shall monitor daily the implementation of the procedures in the Sanitation SOP’s.
