Violation:  416.1, 416.2(b)(3), 416.2(e)(2), 416.4(d)

Approximately 0500 hrs while performing pre-operational inspection after plant forms showed all was inspected and passed the following non-compliances were observed. 1) Numerous bugs were seen flying around in the offal vestibule room. Bugs were also observed behind the loose metal door frame. The metal drain is not properly sealed which allows water to spill out onto the floor. Between the metal drain and the concrete chicken residue and feathers were found from previous day's production and black mold was seen in the pits and cracks of the concrete. Area was rejected and sealed off. Tag # B40954022 was applied. 2) In the cooler at approximately 0630 hrs 4 vats of ice and 2 vats of product that were covered with plastic were found with bugs and black specks on the plastic covering. QC and were notified of the findings. Cooler was rejected and sealed off. Tag # B40954020 was applied. These insanitary conditions are in violation of 9CFR 416.1, 416.2(b)(3), 416.2(e)(2) and 416.4(d). This serves as written notification that your failure to comply with regulatory requirements could result in additional regulatory or administrative action.


416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.

416.2(e)(2) Properly convey sewage and liquid disposable waste from the establishment

416.4(d) Product must be protected from adulteration during processing, handling, storage, loading, and unloading at and during transportation from official establishments.
