Citation: During the Course of the 60 day review the following Noncompliance's were noted by EIAO The same person is monitoring and performing verification for CCP B4,B5, B6, B10, B11, B12. They failed to address Listeria monocytogenes during the addition of doors and the fiberglass walls. This is a violation of 9CFR 417.5(3) which states" Records documenting the monitoring of CCP's and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment's HACCP plan." Mr. was informed of this noncompliance and that continued failure could result in further Regulatory control actions.

Each of these records shall include the date the record was made.
