Violation: 416.1, 416.2(b)(2), 416.2(b)(3)

Citation: At approximately 0612 hours, while walking through Packing Department by the Stack Off area, I observed water dripping on a stack of box product (CVP). At close examination, I observed water dripping from the ceiling down onto boxes of Trussed WOG Broiler. I observed two boxes covers very wet from the water dripping on the boxes of product. I immediately informed (Quality Assurance) of my observation. The product was immediately removed from this area and the two box covers were replace with new covers. Approximately 1105 hours, while walking with (2nd Processing Manager) showing her area of concerns in the Breast Deboning Department, I observed water dripping from the upper frame work to the emergency exit door onto the floor in the deboning department by label scale location. Further examination, I observed water dripping from the ceiling onto the floor approximately two feet from the emergency exit door. I then proceeding to the emergency exit door in the Leg Deboning area, I observed water running on the floor from the bottom portion door (sealed with plastic material). I informed of my observation. At approximately 1525 hours, while walking up the stairway by the knives/scissors sharpening room, I observed water running down the stairway making the stairs very slipping to walk on. At close examination, the exit door the roof had a two inch wide and four feet long open at the bottom portion of the door, causing the water to run under the door frame onto the stairway. I informed of my observation. A check of the plant SSOP's forms, plant's Operational Sanitation records and GMP Monitor's records (Packing and Deboning Department) shows no documentation for the above noncompliance's on 09/30/2010. Establishment management has received oral and written notification concerning this noncompliance. This document serves as a written notification that your failure to comply with regulatory requirement could result in additional enforcement action described in 9 CFR part 500. Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(b)(2) Walls, floors, and ceilings within establishments must be built of durable materials impervious to moisture and be cleaned and sanitized as necessary to prevent adulteration of product or the creation of insanitary conditions.

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.
