Citation: At approximately 0920 hours, while reviewing the HACCP records for CCP # [redacted] for 5/27/2011, I observed that there had been a deviation from the critical limit at 0903 hours. The temperature was documented to be [redacted] degrees F after final grind. The critical limit for this CCP is [redacted]. Also documented at this time was a direct observation of the monitoring as noted by initials. The column for the direct observation states "[redacted] No signature is noted in this column so it is not clear if a recheck was performed. In the column titled " Comments and/or corrective actions" is the entry, "The temperature not good go to freezer." On the back of the form for CCP # [redacted] is a corrective actions form. This form has a place to enter the time of the CCP failure and underneath this are places to enter the corrective actions in places labeled " Step 1" through " Step 4" At the bottom of this form is the statement " Follow corrective actions in HACCP plan." The only entry on this form is the time 9:03 am. When I asked Mr. [redacted] HACCP Manager, if any of the corrective actions in their HACCP plan were taken or documented, he did not know with the exception that the product was taken to the freezer. I asked if the product was still at the establishment and he was unsure. In answer to my question, I was given a Raw Ground Lot Tracking Information form that had a single entry for the date 5/27. Under the column titled "Lot number" was the number 30016. This references to Lot number of the product that was ground not of what was shipped. I went to the area where the product is ground and asked the supervisor how the product was identified. He told me that the Julian date for the date of production is placed on the shipping container. I asked if the product produced was still on the premises and Mr. [redacted] was unable to answer. However, the supervisor said that it was shipped the same day it was produced on 5/27/2011. I asked to see the shipping records and was given the shipping manifests. These manifests showed that the product had been shipped on 5/27/2011 and on 5/31/2011. I reviewed the pre-shipment review for this product and noted that at 1215 and 2300 hours on 5/27/2011, the pre shipment for Raw Ground product produced on 5/27/2011 was signed with the CCP 1B results documented as " ok". Mr. [redacted] was verbally notified of the establishment's failure to take corrective actions following a CCP deviation according to 417.3 (a)(1) 417.3(a)(2), 417.3(a)(3) 417.3(a)(4), 417.3(c), 417.4(a)(2)(iii) and 417.5(a)(3).

Regulation:

417.3(a)(1) The written HACCP plan shall identify the corrective action to be followed in response to a deviation from a critical limit. The HACCP plan shall describe the corrective action to be taken, and assign responsibility for taking corrective action, to ensure The cause of the deviation is identified and eliminated

417.3(a)(2) The CCP will be under control after the corrective action is taken

417.3(a)(3) Measures to prevent recurrence are established

417.3(a)(4) No product that is injurious to health or otherwise adulterated as a result of the deviation enters commerce.

417.3(c) All corrective actions taken in accordance with this section shall be documented in records that are subject to verification in accordance with § 417.4(a)(2)(iii) and the recordkeeping requirements of § 417.5 of this part.

417.5(a)(3) Records documenting the monitoring of CCP’s and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment’s HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made.
