Citation: At approximately 1015 hours, while Dr. [redacted] and I were walking through the plant, we observed the following: - In the box storage room, where there were multiple stacks of waxed boxes that were ready to use as well as stacks of unfolded waxed boxes we observed heavy dust on the blade covers of the three fans and the six vents in the room. Two of the fans were blowing around the room and the over head vents were directly above the stacks of boxes. There was lent and UFM on the walls of this room and debris and paper trash on the floor. - In hallway to the entrance of the maintenance room, there was accumulation of dirt and trash on the floor. Employees who worked in the evisceration department were seen going through this area. - The door to the ice penthouse at the top of the stairs in the hallway that leads to maintenance was ajar, in this room we observed that the window at the right side of the entrance door was missing a screen and the window was open. In addition a window at the back of the room was ajar about 6 inches and had 2 electrical cords coming into the room. We verbally informed Mr. [redacted] the General Production Manager of the non-compliances. The establishment failed to comply with 9 CFR 416.2(a), 9 CFR 416.2(b)(1), and 9 CFR 416.2(b)(3). This document serves as written notification that failure to comply with regulatory requirement(s) could result in additional regulatory action as described in 9 CFR 500.


416.2(a) Grounds and pest control. The grounds about an establishment must be maintained to prevent conditions that could lead to insanitary conditions, adulteration of product, or interfere with inspection by FSIS program employees. Establishments must have in place a pest management program to prevent the harborage and breeding of pests on the grounds and within establishment facilities. Pest control substances used must be safe and effective under the conditions of use and not be applied or stored in a manner that will result in the adulteration of product or the creation of insanitary conditions.

416.2(b)(1) Construction. Establishment buildings, including their structures, rooms, and compartments must be of sound construction, be kept in good repair, and be of sufficient size to allow for processing, handling, and storage of product in a manner that does not result in product adulteration or the creation of insanitary conditions.

416.2(b)(3) Walls, floors, ceilings, doors, windows, and other outside openings must be constructed and maintained to prevent the entrance of vermin, such as flies, rats, and mice.
