Citation: At approximately 1155 hours, CSI [redacted] informed me after sopped production due to finding alive cockroach inside a chicken carcass on line # [redacted] at station #2. The roach was brown in color and half inch in length. The roach was shown to [redacted], Evisceration Department Supervisor. The roach was killed and the product was condemned. The same non compliance happened at the same line and same station on 9/15/2011. This is failure to meet regulatory requirements of 9 CFR 416.2(a), 416.4(d), and 416.13(c). This document serves as a written notification that failure to comply with regulatory requirement(s) could result in additional regulatory control action as described in 9 CFR 500. Past Similar NRs - Previous Ineffective Plant Actions: The wall was treated. work notification was issued to seal the top of the wall with expanding foam. Target for completion 10/02/1. NR: 89-2011 dated 9/15/2011


416.13(c) Each official establishment shall monitor daily the implementation of the procedures in the Sanitation SOPís.

416.2(a) Grounds and pest control. The grounds about an establishment must be maintained to prevent conditions that could lead to insanitary conditions, adulteration of product, or interfere with inspection by FSIS program employees. Establishments must have in place a pest management program to prevent the harborage and breeding of pests on the grounds and within establishment facilities. Pest control substances used must be safe and effective under the conditions of use and not be applied or stored in a manner that will result in the adulteration of product or the creation of insanitary conditions.

416.4(d) Product must be protected from adulteration during processing, handling, storage, loading, and unloading at and during transportation from official establishments.
