Citation: At approximately 1619 hours, while performing a 01C02 inspection task in the ingredients cage. As I was walking through checking the ingredients on the pallets, I slipped on a five foot by 3 foot area of black sludge in between the pallet holding the light brown sugar and a pallet breader, containing an allergen dry ingredient. Upon further investigation, I was able to determine that the drops of water were coming from ceiling beam adjacent the yellow funnels that were set up to collect pre-existing leakage. Maintenance Supervisor, [redacted] was paged and reported to area first. As I further surveyed the immediate area, I noticed on the allergen labeled pallet that a bag in the third row from the top was opened on the edge and some of the contents were on the floor, thus creating insanitary conditions. Then second shift assistant plant manager, [redacted] was notified of the deficiencies and a noncompliance record would be written. The establishment took corrective actions. It was verified that the allergen containing ingredient was properly disposed of.

Regulation:

416.2(b)(2) Walls, floors, and ceilings within establishments must be built of durable materials impervious to moisture and be cleaned and sanitized as necessary to prevent adulteration of product or the creation of insanitary conditions.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
