Citation: Today 10/10/11 at approximately 0530 to 0545 hours while performing pre-operational sanitation inspection prior to the start of operations and after the Establishment 791 pre-operational sanitation inspection, I observed the following non-compliances in Room 2495 (Kill Floor): In Unit 35 I observed approximately 10 to 12 drain flies on auger wall, split saw shield and the east wall. In addition, in Unit 10 I observed grease specs on 2 white knife sharpeners. These areas were cleaned and sanitized immediately before continuing inspection. The above mentioned equipment would be considered non product contact surfaces. In addition while performing pre-operational inspection in Room 1285 (Offal Room) I observed the following non-compliances: in Unit 4 I found pieces of meat in 2 sterilizers (product contact surface) as well as fat and meat scraps on underside of some cutting boards. In Unit 20 I observed 15 to 20 dead drain flies combined in stainless steel tubs used for livers and kidneys. In Unit 4 I observed heavy fat and meat build up inside the stomach washer. All of the above mentioned equipment are direct product contact surfaces. No USDA/RETAIN tag was applied because these items were cleaned and sanitized under my direct observation. After reviewing Establishment 791’s Pre-Operational Sanitation Records I found no documentation denoting the deficiencies mentioned. Dr. Outsource/HACCP Coordinator was verbally notified of these noncompliances. and the failure to meet regulatory requirements 9CFR 416.1, 416.2(a), 416.4(a), 416.4(b) as well as the Establishment 791's SSOP dated 4/11/11 which states on Page 3 .[redacted]" Also GMP's dated 6/6/11 which states on page 9 "Effective measures will be taken to exclude pests from the food processing... areas to protect against contamination of food on premises by pests."

Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(a) Grounds and pest control. The grounds about an establishment must be maintained to prevent conditions that could lead to insanitary conditions, adulteration of product, or interfere with inspection by FSIS program employees. Establishments must have in place a pest management program to prevent the harborage and breeding of pests on the grounds and within establishment facilities. Pest control substances used must be safe and effective under the conditions of use and not be applied or stored in a manner that will result in the adulteration of product or the creation of insanitary conditions.

416.4(a) All food-contact surfaces, including food-contact surfaces of utensils and equipment, must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
