Citation: On Tuesday April 24,2012 approximately at 0730 hours, I found the following non compliance: while exiting the USDA rest room on the second floor, I saw a cockroach walking on the floor inside the rest room. I kept the cockroach and I showed it to QC Personnel who was informed with the non compliance. The establishment is not in compliance with 9 CFR 416.1 which states : Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated. And 9 CFR 416.2(a) which states: The grounds about an establishment must be maintained to prevent conditions that could lead to insanitary conditions, adulteration of product, or interfere with inspection by FSIS program employees. Establishments must have in place a pest management program to prevent the harborage and breeding of pests on the grounds and within establishment facilities. Pest control substances used must be safe and effective under the conditions of use and not be applied or stored in a manner that will result in the adulteration of product or the creation of insanitary conditions. The non compliance was linked to NR# YKM4612020422N dated 2/22/2012. Previous corrective actions were not effective or were improperly implemented to prevent recurrence.

Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(a) The grounds about an establishment must be maintained to prevent conditions that could lead to insanitary conditions, adulteration of product, or interfere with inspection by FSIS program employees. Establishments must have in place a pest management program to prevent the harborage and breeding of pests on the grounds and within establishment facilities. Pest control substances used must be safe and effective under the conditions of use and not be applied or stored in a manner that will result in the adulteration of product or the creation of insanitary conditions.
