Citation: On 12 July 2010, at approximately 1350 hrs, whilst performing an unscheduled ISP procedure code 03H01, a review of USA-HACCP-FORM-009, also known as the "Chilling Record", revealed that there was no entry in the box for "Temp." for CCP 1B: Verification Activities. These were dated 2:17 pm on 09 July 2010, the results of the monitoring procedures were deemed to be acceptable, and the verification was signed as being performed by [redacted] QC, was notified of the noncompliance by approximately 1353 hrs. No regulatory control action was taken as no product was directly affected. This 03H01 procedure was followed by an 03H02 procedure, and no additional deficiencies were discovered. The establishment is not in compliance with the following regulations: 9 CFR 417.4(a)(2)(ii) & (iii), which state the following: "Ongoing verification activities. Ongoing verification activities include, but are not limited to: (ii) Direct observations of monitoring activities and corrective actions; and (iii) The review of records generated and maintained in accordance with 417.5(a)(3) of this part."; 9 CFR 417.5(a)(3), which states the following: "Records documenting the monitoring of CCP's and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment's HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made." This document serves as written notification that your failure to comply with these regulatory requirements could result in additional regulatory or administrative action.

Each of these records shall include the date the record was made.
