Citation: At 0920 hrs this morning Dr. [redacted] informed me that during dispositions he observed an employee pushing carcass numbers MPD474451128 and MPD47451131 into the carcass cooler without applying the intervention of peroxyacedic acid. General Manager [redacted] was informed of this and he then informed the employees to do it. Dr. [redacted] directly observed this being performed and released them. You re in violation of regulations 9CFR417.6 that states "A HACCP system may be found to be inadequate if: (a) The HACCP plan in operation does not meet the requirements set forth in this part; (b) Establishment personnel are not performing tasks specified in the HACCP plan; (c) The establishment fails to take corrective actions, as required by 417.3 of this part; (d) HACCP records are not being maintained as required in 417.5 of this part; or (e) Adulterated product is produced or shipped." and regulation 9CFR417.(c)(4) states "(4) List the procedures, and the frequency with which those procedures will be performed, that will be used to monitor each of the critical control points to ensure compliance with the critical limits; Past Similar NRs - Previous Ineffective Plant Actions: Monitoring of the CCP NR: 18-2010 dated 3/17/2010
