Citation: At approximately 0815 hours while performing scheduled ISP code 01B02 on the kill floor I observed the following deficiencies. There are several dead flies in the florescent light cover tubes over the head wash cabinet. The cardboard that is used for covering the table next to the head wash cabinet is dirty and stained. The doors of the electrical cabinet above the table have residue build up on them. There is a glue like substance on the wall junction located next to the chill cooler. There is also some residue build up on the surrounding walls and ledges. The metal flashing around the door leading to the inedible room has separated and exposed a gap about 4 inches by 6 inches. A check of the daily SSOP records found that these deficiencies were not documented. This is a violation of FSIS regulations 416.1 and 416.4(b). I notified Mr. [redacted] orally and in writing with this NR of the noncompliance.


416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
