Citation: On 10/4/10 at approximately 0745 hours while performing procedure code 01B02 pre-operational SSOP I observed the following deficiencies . 1. carcass splitting saw was dirty with meat and fat particles . 2. sterilizer was operational and was dirty with a brown froth on top of the water and the sterilizer had a unknown buildup . 3. knock box counter balance had dried blood and meat and fat particles . 4. kick plate by cooler door had an accumulation of meat and fat residue behind plate . 5. wooden rail support had dried blood . 6. employee break room / locker room had cigarette butts and various debris on floor , various clutter throughout , clothing , boots rags etc . A regulatory control action was taken on the break/locker room using US Reject tag #B38685282. There was no indication that these noncompliance's were observed by the establishment . Failure to control this type of noncompliance's could result in further regulatory and or administrative control measures. Past Similar NRs - Previous Ineffective Plant Actions: NR 's had not been addressed by establishment , no effective preventitive measures instituted NR: 43-2010 dated 9/13/2010; NR: 41-2010 dated 9/10/2010

Regulation:

416.4(a) All food-contact surfaces, including food-contact surfaces of utensils and equipment, must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
