Citation: At approximately 2140 hours while performing an unscheduled 01C02 procedure task I observed the following: -An approximate 6' X 3' X 2" area of stagnant poultry juices/ water in the center of the Cooler adjacent to the entrance to the Freezer (employee traffic area). -A soiled broken wooden pallet plank stored in a trash receptacle punctured the protective covering to a bag of ice stored on a pallet on the right side of the Freezer and is in direct contact with the exposed ice used to chill various exposed chicken products. One pallet consisting of 70 bags of ice rejected with US Rejected tag #B40 954048. This is a failure to meet 9CFR 416.1; 416.2(e)(4) and 9CFR 416.4(d). Processing Co-Manager, and HACCP/QC Manager, [redacted] were verbally notified immediately of the non-compliances and actions taken. This document serves as written notification that your failure to comply with regulatory requirement(s) could result in additional regulatory or administrative action.

Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(e)(4) Provide adequate floor drainage in all areas where floors are subject to flooding-type cleaning or where normal operations release or discharge water or other liquid waste on the floor

416.4(d) Product must be protected from adulteration during processing, handling, storage, loading, and unloading at and during transportation from official establishments.
