Citation: Today at approximately 0705 hours before the operation started, and after the plant manager Mr. [redacted] said that he had finished his Pre-Op task, and found all areas were acceptable. I performed my Pre-Op inspection and found the following: (1) The right hand side wall of the evisceration room appearing with many black color foreign particles, and black color residues appearing along at the edges of the wall panel sidings and at around the wall panel mount plastic button-head nails. (2) Black mold appearing at the junction of the ceiling and the wall with a clock in the evisceration room. (4) Rust and residue appearing at the top of the door entrance of a cooler next to the slaughter room. The plant manager Mr. [redacted] was notified and shown of the noncompliance.
