Citation: At approximately 1025 hours, while conducting the recordkeeping component of an unscheduled 03J02 procedure task I observed the following; no direct observation performance or documented direct observation performance of the chlorine level check, CCP#2B (Final Wash Cabinet), on HACCP Form 2B the date of 5/23/11. The only direct observation performance conducted by the establishment and documented on HACCP Form 2B the date of 5/23/11 was performed on the chlorine level check that is identified in the establishment's pre-requisite program used as supporting documentation for its Raw Not Ground HACCP program. The establishments Slaughter HACCP plan signed and dated 5/5/11, page 2 under Verification Procedures and Frequency states: "[redacted] ". I immediately notified HACCP Coordinator, [redacted] verbally of the non-compliance. This is a failure to meet: 9CFR 417.5(a)(3) and 9CFR 417.4(a)(2)(ii). Past Similar Nr's-Previous Ineffective Plant Actions: Have re-instructed production supervisor that he must perform the 10 bird inspection prior to evis being completed. If he identifies contamination he must check another 10 carcasses, after insuring the contaminated carcass was re-cleaned. Also that the D.O must be performed during one of the checks. NR: 17-2011 dated 5/17/11. The 02 procedure will be performed.

417.5(a)(3) Records documenting the monitoring of CCP’s and their critical limits, including the recording of actual times, temperatures, or other quantifiable values, as prescribed in the establishment’s HACCP plan; the calibration of process-monitoring instruments; corrective actions, including all actions taken in response to a deviation; verification procedures and results; product code(s), product name or identity, or slaughter production lot. Each of these records shall include the date the record was made.
