Citation: At 0635 hours while performing a scheduled 06D01 PBIS procedure with my Supervisor, to verify that Est. # 18860 was meeting the regulatory requirements, I observed the following non-compliant conditions; 1)Mold and mildew on the Employee Bathroom wall and ceiling 2)Insanitary aprons hanging from a nail on the wall in rear storage area 3) Inedible cooler storage door, there was rust and the identification sign was illegible 4)In the hallway connecting the live bird receiving/holding area to the kill room there is a hole along the juncture of the floor and the wall. 5)The entire door jamb surrounding the portal between the kill-scald area, is painted and the paint is peeling and blistered. 6) Overhead metallic air condition blower unit affixed to the ceiling directly above the eviscerating work table shows pitting and scaling paint I notified the floor supervisor, [redacted] and took regulatory control action by applying US Reject Tag No. B37056547 to the affected areas. These non-compliant conditions are in violation of Reg 416.4(b) for non-food contact surfaces. This NR is linked to NR # 2-2010 dated 7/29/2010. Management was informed that continued failure to comply would result in further enforcement actions. Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.

416.4(d) Product must be protected from adulteration during processing, handling, storage, loading, and unloading at and during transportation from official establishments.
